Intellectual Disabilities

CLHSD’s Intellectual Disabilities Office serves as the Single Point of Entry for individuals and their families seeking supports or services through the Louisiana Department of Health. This includes individual and family support for people with intellectual and developmental disabilities living in their own homes or with their families in their own communities. Resources include personal care services, extended care, companion care, dental and medical services, equipment and supplies, communication services, crisis intervention, and specialized nutrition. Support Coordinators provide information, assessment, plan of support, referrals, and ongoing coordination and monitoring. Flexible Family Fund provides a monthly stipend to families of eligible children with severe or profound developmental disabilities from birth to age 18.  Waiver Services are offered based on needs of the individual.  A Screening for Urgency of Need (SUN) is administered to determine the need for waiver services.  Individuals with an Emergent or Urgent need will receive an offer for the most appropriate waiver.
